加國醫療現狀 - 見仁見智
萬維今日刊登一篇對加拿大醫療體制大有評判的文章:《健康所系 性命相托--不得不說的加國醫療現狀》，本來以為我要說話都說完了，可現在又不得已拿起筆，還得再說幾句話。 首先說明本人不是加拿大醫療體制的專業辯護者。可是我對此文的片面觀點不敢苟同。比如文章里有這一段話： 。。。加拿大一直以優越的全民醫保制度自傲於全球，也是吸引我們選擇移民的原因之一，然而這一健保體系具有一定的封閉性和“社會主義”色彩，醫療福利過多集中於老幼婦女及殘障人士等，故而加拿大可稱為"養老天堂"。。。 這話既有“戴着彩色眼鏡看東西”之嫌，亦有誤導的作用。 文章還說： 。。。在所有採用相似醫保制度的國家中，加拿大是人均花費最高的。。。 這句話不符合事實。2010年www.visualeconomics.com 發表根據路透社的研究結論(#)已經反駁這個說法。 文章說的家庭醫生“包醫百病”也不正確。謹慎的FP (Family Doctor) 通常把特殊症狀轉診於專科醫生。然後專家按症狀的嚴重性給你預約日期。當然那嚴重性是專家判定，而不是你判定。這是爭議的焦點。有錢人可以“判定”自己症狀的嚴重性，掏出錢包去美國即刻買到專家無微不至的照顧，而政府“贊助”的加拿大人只有聽從專家指示的選擇。 文章里說的那些“怪”現象，本人和家人從來沒有遭遇過。對加拿大給我們的醫療，雖然有可以改善之處，大致還是褒多於貶。一般加拿大居民也有同感。根據最新民意調查，85.2% 15歲或以上的加拿大對他們的醫療保障表示滿意。(*) 我認為獲取高效果的醫療，關鍵在於和家庭醫生和醫療人員（包括診所的護士和接待小姐們）保持友善和相互尊重的關係。而且病人必須學會成為自己的擁護者(advocate)，這不是說要有對抗性的擁護態度，而是要學好和醫生有效地溝通，對這一點英語會話能力甚為重要。 是的，加拿大醫療體制絕不是包治百病的萬靈藥，其不足之處容易枚舉。可是世界上沒有一個國家可以說，“我們的醫療體制是完善的”，定義上“單付款人”體制醫療有不完善之內涵意義，可是“用戶自付”體制也有其不完善之處。有財富的人可以用金錢買得到最先進的醫療，但在加拿大這不是現實的普世標準。對絕大部分加拿大人來說，當前的體系是最現實，是他們想象和盼望中最能達到，最負擔得起的體系。 最後讓我引用一位加國報紙讀者所說的話做個總結： “(In your criticism) are you looking for something in particular or just trying to discredit the results? The survey acknowledges a continuous shortage of family doctors and nurses. We all know the service is not perfect, but it does seem to be getting better. There are pockets where it is great and some not so great but you will have that regardless of the type of system in place. I had the "pleasure" just this last weekend of having to use our health care system and found the quality to be quite acceptable. As a precaution to my condition I was kept under observation for 6 hours after admission and all the tests performed a second time as a precaution.
