4. Proof of Vacation by your employer
顧主出具休假證明

5. Previous record of visiting Mainland China or former Chinese passport
曾經訪問過中國大陸的簽證記錄或原中國護照

6. Completed Visa Application Form (Section must be completed)
完整填寫的簽證申請表(表中第項必須填寫)

2. Certificate of Financial Guarantee(such as latest banking certificate or statement, letter of sponsorship, etc.)
經濟擔保證明 (如最新的存款證明或每月的存款單 擔保函等)

3. Invitation letter from your relative or friend in Mainland China (original or fax copy). (It must specify your name; name, address & telephone number of the sponsor; relationship between the two; duration and the same address of your stay)
在中國大陸的親友的邀請函 原件或複印件
(該邀請函必須包含您的姓名; 邀請人的姓名、住址、聯繫電話；對方關係和您何時住在該址跟保證等)

4. Your Marriage Certificate or Notarized Birth Certificate (a copy is acceptable)
您的結婚證或結婚公證公證書 (可提供複印件)

5. Your Birth Certificate or Notarized Birth Certificate (a copy is acceptable)

(該邀請函必須包含您的姓名; 邀請人的姓名及職務、所屬公司的名稱，地址和聯繫電話；訪問時間何目的，以及您住在該址的保證等)

4. Confirmation letter from your employer in the United States
(The letter must specify the applicant’s name & position, the employer’s name & position, the company’s name, address & phone number, also purpose, time duration and person or company to visit, and guarantee for expenses in China, etc.)
美方雇主公司的證明信
(該證明信須包含申請人的姓名和職務，雇主的姓名及職務，所屬公司的名稱，地址和電話，訪問對象，時間和目的以及訪問費用擔保等)

5. License of your company
公司註冊註冊證明

6. Proof of cooperation between your company and Chinese counterpart (such as agreement of intention, contract of deal, etc.)
與中國公司業務往來證明 (如合作意向或購買協議等)

7. Original Visa Notice from an authorized department of China (by provincial or municipal foreign affairs office or economic & trade office)

8. Previous record of visiting Mainland China or former Chinese passport
